- Trip Registration Forms are to be completed by clubs to inform the Sports Union of your clubs activities, in the event of an accident. It also ensures that your trip is covered by the Sports Unon insurance - is used as a confirmation of your clubs trip as an SU activity.
- Forms are only available from the SU Office at this point in time - so please pop into Level 7 to pick one up!
- On completing the form, pass the WHITE copy to the Sports Development Officer (

- On the morning of the trip, please note make any necessary amendments to the form which have occurred inbetween handing a form into the SU and going on your trip. This could be members who have been unable to attend, venue change due to adverse weather etc.
- Detach the BLUE copy and place in the club pigeon hole, or pass to the Sports Dev Officer in Level 7.
- The YELLOW copy can be kept for your own records.
