SU Forms

The Sports Union has a number of FORMS available which clubs are required to complete. Please click on the relevant links below to access information on completing the forms, under the following headings:

BUCS/SUS/EVENT FUNDING FORM (Yellow) - used when entering a BUCS or SUS event.
TRANSPORT FORM (Purple) - used when hiring any vehicles from the SU.
TRIP REGISTRATION FORMS (white/yellow/blue) - used to confirm trips for insurance purposes.
CASH CLAIM FORM (Blue) - used when claiming for cash which has been paid out by a club.
CHEQUE REQUEST FORM (Pink) - used when claiming for affiliation fees, entry fees, accommodation.
PURCHASE ORDER REQUEST (White) - used to request a PO number when you wish to order any new kit, equipment.
CLOTHING FORM (Orange) - used when ordering leisure wear for clubs.
CASH/CHEQUE DEPOSIT FORM - used for depositing funds into your club account.
AWARDS APPLICATION FORMS - used for nominating club members for sporting/voluntary achievements.

Paper copies of the forms are kept in the SU Office, Level 7 The Union building. Pop in any time to complete the forms - we can give you advice on how to complete them should you require any assistance.
